Registration: Pre-registration is now closed. However, your friends and colleagues can still register onsite!
Onsite registration will open at 11:30 a.m. on Tuesday, April 5, 2022. Please share the onsite registration form and have them bring it to the registration desk located in the Grand Foyer of the Napa Valley Marriott Hotel and Spa. FDAC Attendee Roster: Registered attendees of the FDAC Annual Conference will be provided with an attendee roster listing name, agencies, phone numbers and email addresses
of all registered conference participants. Attendees wishing to opt out of having their information printed in the roster may do so during the registration process. Attendees registering after March 24, 2022 will not be listed in the 2022 FDAC Annual
Conference attendee roster.
Cancellation Policy: Cancellations notices must be made IN WRITING and received by FDAC no later than March 25, 2022. All cancellations received
on or by March 25, 2022 will be refunded $50 less to cover processing fee. There will be no refunds for cancellations made after March 25, 2022. Please send cancellation notices to staff@FDAC.org.
Refund exceptions may be considered on a case-by-case basis for registered attendees should they, or a member of their immediate household, test positive for COVID-19 after March 25, 2022. Please contact Carmen Berry, cberry@FDAC.org,
for assistance.
COVID-19 Personal Responsibility and Liability Waiver: By registering for, or attending, the FDAC Annual Conference you voluntarily assume all risks
and accept sole responsibility for any illness or injury related to exposure to COVID-19, and release FDAC from liability for any injury or harm that may result from attending the FDAC Annual Conference. FDAC's goal is to ensure a safe meeting environment.
As such, you also agree to abide by any prevention or safety measures that FDAC, the Napa Valley Marriott, the Napa County Health Department or Center for Disease Control may have in place during the event. You also agree not to attend the FDAC Annual
Conference if you become ill, are aware that you’ve been directly exposed to the COVID-19 virus within two weeks prior to the Event, or exhibit any symptoms of the COVID-19 virus prior to or during the event.

Additional Information: The registration fee includes snacks at the Pre-Conference on Tuesday, a continental breakfast each day Wednesday through Friday and

Conference dress attire is business casual. It is recommended that you have a jacket or sweater, as the ballroom is kept at a cool temperature. Attendees requiring ADA accommodations should contact Carmen Berry, cberry@FDAC.org,
to ensure that appropriate arrangements are made prior to the event.
